DMC Membership Application
Click on the membership application link to submit your new DMC
membership electronically (or renew).
This form
can now be submitted electronically and we can accept your payment
immediately online via PayPal!
When filling out the form, if you wish to pay via PayPal, choose
"PayPal-Electronic" in the Payment box and click on the red box and your
web browser will open to our payment webpage, prefilled with the $25
payment amount for a new/renewal annual membership.
Once you complete the payment, make sure you return to the PDF form and
complete it and then click "Submit" to send your completed application.
If you still wish to pay via check, and mail us the form, you may still
do that as well. Fill out the application, print it and mail it to

But why not just take care of it electronically now, and save the stamp?
Please note, this form does require that you use the Adobe Acrobat
Reader, available for free at
www.adobe.com So if you are having problems filling out the form,
please make sure your Adobe Reader is the current version.
Within a few days you will receive, via email, a welcome letter with
additional information about how to access the Members Only section of
the discussion forum.
You will also receive a DMC Membership card and welcome letter via paper
mail.
